Each building will have paper packets available for pick up starting 7/21/25. Please complete all forms and return them to the school office prior to 7/31/25. If you have moved over the summer please provide a new proof of address.

- Proof of Address: Electric, gas or water bill, rental or lease agreement
- Birth Certificate
- Social Security Card
- Well Child Physical
- Kindergarten Students Only
- Athletes must have a current physical
- Immunization Record
