Nurses from the Lonoke County Health Department will be on the Lonoke High School campus on Tuesday, October 18, 2022 from 3:00-6:00 p.m., during Parent/Teacher Conferences.
They will be providing flu shots to LPSD students ONLY. Parents/Guardians will not be able to receive flu shots at this clinic. Students must bring their consent form and give it to the nurse at the flu clinic. The nurses will be set up between the Gina Cox Center and the visitor's bleachers of the football stadium. Please see the map for drive-thru directions.
Please fill out the FERPA Consent Form and the Immunization Consent Form ahead of the Student Flu Shot Clinic.
